This membership is offered by Doctors’ Choice Awards - DCA Association for a period of 1 year from the date the agreement is accepted and entered into.

You will be entitled to a list of exclusive benefits. Please visit the Perks and Privileges page to learn more.

During the 1 year membership period you are expected to live up to the commitments and grow personally as a Doctor's Doctor and help other doctors grow in this regard.

We will review whether you are living up to the commitments on a semi-annual basis and have the right to cancel your Membership if you are not living up to your commitments.

Please note this membership and its benefits are offered complimentary and you do not have to pay for these privileges.

I Dr. hereby apply for admission to membership of Doctors' Choice Association. I hereby undertake, if admitted to membership, to be bound by the commitments of the association:

1. I will rate and review atleast 5 other doctors with my honest opinion every year.
2. I will personally grow as a Doctor's Doctor and help other doctors as well.
